Translation pränataler Ultraschallbefunde in postnatales Outcome - Optimierung des individuellen Managements von Patienten mit posterioren Urethralklappen (PUV)

Zusammenfassung der Projektergebnisse

Obstruktionen des unteren Urogenitaltraktes (LUTO) treten bei 2-3 von 10000 männlichen Neugeborenen auf und sind mit hohen Mortalitätsraten bis 30% verbunden sowie mit signifikanten Begleiterkrankungen wie pulmonaler Hypoplasie, Blasendysfunktion und chronischer Niereninsuffizienz, von denen ein Drittel eine terminale Niereninsuffizienz entwickelt. Die häufigste zugrundeliegende Ursache mit 1-2 Betroffenen in 10000 männlichen Geburten stellen posteriore Urethralklappen (PUV) dar, die das Ergebnis einer unvollständigien Inkorporation des Wolff’schen Ganges sind, gefolgt von Urethralatresie, Urethralsteonose/-striktur. Die frühzeitige pränatale Detektion von LUTO, insbesondere im 1. Trimester der Schwangerschaft, ist mit einer schlechten Prognose assoziiert. Eine pränatale Diagnosestellung ist bei Vorliegen typischer Ultraschallbefunde wie Megazystis, Hydronephrose und dem „keyhole sign“ offensichtlich. Allerdings erlauben diese Befunde keine Rückschlüsse auf die zugrundeliegende Ursache von LUTO oder eine genaue Risikoabschätzung bezüglich des postnatalen Outcomes oder der Wahrscheinlichkeit einer schlechten Prognose und die Detektionsraten in der Literatur liegen nur zwischen 33-62%. Obwohl die pränatalen Detektionsraten in Toronto etwas im oberen Bereich dieser Spanne liegen, sind sie weiterhin nicht optimal. Die Ungewissheit der postnatalen Prognose sowie der zugrundeliegenden Diagnose stellen gleichermaßen eine Herausforderung für den Behandelnden als auch die werdenden Eltern dar, die konfrontiert mit abstrakten klinischen Parametern und Annahmen auf geringem Evidenzlevel versuchen, die daraus erwachsenden Konsequenzen für ihr zukünftiges Leben &, das des Föten abzuleiten. Daher entscheidet sich nahezu die Hälfte dieser Familien dazu, die Schwangerschaft abzubrechen. Aus diesem Grund, haben wir uns gefragt, ob eine personalisierte pränatale Risikoabschätzung basierend auf pränatalen Ultraschallbefunden individueller Pateinten möglich ist, um postnatale Outcomes wie die Wahrscheinlichkeit von Dialyse, Transplantation oder Tod von LUTO-Föten vorhersagen zu können, um Eltern in ihrer Entscheidungsfindung bezüglich der Schwangerschaft zu unterstützen. In Zusammenarbeit mit einem high-risk Fetalzentrum wurde im Rahmen dieses Projekts eine pränatale LUTO-Datenbank erstellt und anschließend mit der bereits existierenden postnatalen Datenbank des tertiären Kinderzentrums gematcht. Das aus diesen Trainingssets konstruierte Random Forest Model wurde anschließend zunächst intern validiert an terminierten LUTO-Schwangerschaften. Die ersten Ergebnisse sind vielversprechend und demonstrieren, dass es möglich ist, anhand von pränatalen Ultraschallbefunden das postnatale Outcome von LUTO-Föten vorherzusagen und mit diesem Tool die pränatale Beratung zu optimieren mittels personalisierter Risikostratifizierung.
