Therapy of c/UICC stage II/III rectal cancer of the upper third by quality controlled total-versus partial mesorectal excision, followed by adjuvant chemotherapy

Final Report Abstract

In der randomisierten, multizentrischen GAST-05-Phase-IIb-Studie wird bei 372 Patienten (f: 35,2%; m: 64,8%; Alter: 66 ± 10,7 J.) mit einem lokal fortgeschrittenen Adenokarzinom (LARC) im oberen Rektum (≥ 12 cm proximal der ACL) gezeigt, dass eine qualitätskontrollierte partielle mesorektale Exzision (PME) der bisher etablierten totalen mesorektalen Exzision (TME) onkologisch gleichwertig ist (Lokalrezidiv-Rate nach > 5 J. Nachbeobachtung: 4.7%). In den sekundären Endpunkten, wie z. B. der Komplikationsrate und Krankenhaus-Verweildauer, war die PME der TME (signifikant) überlegen. Außerdem kann bei den Studienteilnehmer:innen der GAST-05- Studie erstmals dargestellt werden, dass mit einer postoperativen (adjuvanten) FOLFOX-Chemotherapie für LARC-Patienten im Stadium UICC-III signifikante Überlebensvorteile erreichbar sind.
