GRK 1034:  Die Bedeutung genetischer Polymorphismen in der Onkologie: Von den Grundlagen zur individualisierten Therapie

Zusammenfassung der Projektergebnisse

Etwa 500.000 Menschen erkranken jährlich in Deutschland an Krebs und etwa jeder vierte Mensch stirbt schließlich an einer Krebserkrankung. Fehlendes Ansprechen auf die Krebstherapie und schwere Therapie-Nebenwirkungen sind auch heute noch ernste Probleme in der Behandlung. Das Ansprechen auf die Krebstherapie hängt oft von den Mutationen ab, die sich während der Entstehung der Krebsentstehung in den Krebszellen ansammeln. Die individuelle Belastung durch Nebenwirkungen hängt dagegen oft mit der angeborenen Variation im Erbgut, also im Genom der normalen Zellen des Menschen, zusammen. Aber wir verstehen viele dieser Zusammenhänge noch nicht genug, um dieses Wissen regelmäßig in der Behandlung zum Nutzen für die Patienten berücksichtigen zu können. Zusammen mit vielen anderen Wissenschaftlerinnen und Wissenschaftlern haben die 69 Doktorandinnen und Doktoranden des Graduiertenkollegs 1034 daher untersucht, wie die Wirkweise und Wirkstärke und die Nebenwirkungen von Krebstherapien mit der angeborenen genomischen Variation beim Menschen zusammenhängen. Die Vorgänge in den menschlichen Zellen ebenso wie die therapeutischen Maßnahmen müssen im Zusammenhang aller Organsysteme, des gesamten Menschen und der jeweiligen Fachdisziplinen betrachtet werden. Daher haben im Graduiertenkolleg Forschungsgruppen aus der Grundlagenforschung mit klinischen Disziplinen der Krebstherapie (Hämatologie, Onkologie, Chirurgie, Dermatologie) eng zusammengearbeitet. Die Doktorandinnen und Doktoranden haben die genomische Variation in der Pharmakologie (Membrantransport, Metabolismus von Medikamenten und Rezeptorwirkungen), in der Genregulation, in der DNA-Reparatur, in der Immunologie, und in der Gefäßneubildung untersucht. Und schließlich wurde hier auch an verbesserte Verfahren der bioinformatischen und epidemiologischen Analyse der erblicher Variation bei Krebserkrankungen gearbeitet. Wir haben die Frage bearbeitet, wie Krebsmedikamente überhaupt in die Krebszellen gelangen, wo sie schließlich ihre Wirkung entfalten können. Dabei konnten Doktoranden aus der Physiologie, der klinischen Pharmakologie und der Chirurgie zeigen, dass jeweils einzelne Einwärts-Transportproteine (organische Anionen- und Kationentransporter) für spezifische Medikamente in der Krebstherapie wichtig sind. Unsere Untersuchungen ergaben, dass Krebszellen sehr unterschiedlich mit diesen Transportproteinen ausgestattet sind, was in Zukunft für die individuelle Therapieauswahl eine große Bedeutung haben kann. Die Regulation zwischen Zellen und innerhalb der Zellen spielt in der Krebsentstehung und Behandlung eine zentrale Rolle. Daher bearbeiteten viele der Doktorandinnen und Doktoranden die Genregulation im Immunsystem und in bösartigen Lymphdrüsenkrebserkrankungen. Es zeigten sich spezifische Muster der Dysregulation der Signalwege in den Krebszellen, was wiederum zu individualisierten Therapien beitragen kann. In einer auf die Analyse der Immunabwehr ausgerichteten Projekt konnte gezeigt werden, dass eine Genvariante im stressinduzierbaren NK-Rezeptor-Liganden MICA mit einem erhöhten Risiko der sehr schwerwiegenden Abstoßungsreaktion (graft versus host Erkrankung) nach Knochenmarktransplantation assoziiert war. Eine ebenso wichige Rolle spielt die Reparatur von Erbsubstanz-(DNA)-Schäden in der Krebsentstehung und Therapie. Wir haben neue DNA Reparatur-Genvarianten identifiziert und eine Modulation der DNA-Reparaturfähigkeit als Risiko für die Entwicklung von Krebserkrankung als auch für das Ansprechen und die Nebenwirkungen einer medikamentösen Tumortherapie aufgezeigt. Die Ergebnisse verdeutlichen die sehr erfolgreiche interdisziplinäre Zusammenarbeit aller Mitglieder des Graduiertenkollegs 1034 mit dem Ziel, vielschichtigen Mechanismen der Krebsentstehung, aber auch der individuellen Behandlung von Krebserkrankungen zum Wohle der Betroffenen besser zu verstehen.
