Wirkmechanismen anodaler Gleichstromstimulation bei Patienten mit wiederholten Schädelhirntraumata - Eine multimodale Analyse mittels Elektrophysiologie, MR-Spektroskopie und funktioneller MRT

Zusammenfassung der Projektergebnisse

As patients with mild traumatic brain mTBI generally reveal no overt structural brain lesion on routine magnetic resonance imaging (MRI) or computed tomography (CT) scans, microstructural and neurochemical changes have been proposed to account for slight but persistent deficits in this population. Kobori and Dash (2006) reported a general elevation of gamma aminobutyric acid (GABA) concentration hours after TBI in rats. Transcranial magnetic stimulation (TMS) studies indicate increased GABAB receptor activity in patients after recurrent mTBIs. Our aim was to evaluate differences in cognition, GABA concentration and GABAB receptor activity between healthy controls and patients in the chronic phase after recurrent mTBIs and whether non-invasive brain stimulation may have an effect on GABA concentration and GABAB receptor activity in patients in the chronic phase after recurrent mTBI compared to age-matched healthy controls. We found baseline differences in cognition between patients in the chronic phase after recurrent mTBI and healthy controls. GABA concentration at baseline was associated with the number of mTBI, although no group differences in GABA concentration and GABAB receptor activity were found. In sum, our data did not support a general effect of atDCS on GABA concentration and GABAB receptor activity. Moreover, no modulation of GABA concentration or receptor activity by atDCS was detected, in contrast to several previous studies. Specifics of experimental design and analysis but also characteristics of the respective samples may account for these differential findings and should be addressed in future larger studies. Given the high prevalence of mTBI and previous reports on an association with higher incidence of AD, additional techniques to improve neuroplasticity should be investigated.
