Aseptical loosening at total knee arthroplasty and relative fields.

In the year 2015 the Research Fellowship at Hospital for Special Surgery should be attend to aseptical loosening after total knee arthroplasty (TKA) and possible factors of influence. The aim is to find possible factors that account for early loosening. To this, inter alia, following application area should be examined: loosening and translucent lines at x-ray at the basis of pre- and postoperative varus- and valgus-malalignment.At previous publications a physiological postoperative alignment of the lower limb after TKA for a longer life of the TKA without loosening is postulated. Current publications have shown that the preoperative malalignment is an important factor of influence as well. At the context to these findings other possible factors of influence are not cleared at the common opinion of clinical research (Inlay of TKA in correlation of preoperative alignment, patient satisfaction dependent of pre- and postoperative alignment, postoperative stability of TKA in correlation to preoperative malalignment).Other smaller issues, which should be dealed with at the Research Fellowship, are not totally cleared at the majority scientific opinion. To this, following questions should be cleared: is a tourniquet during implantation of a TKA necessary and does a tourniquet influence the postoperative result (op-time, infection, alignment?). Does op-time influence the frequency of infection? The aim of continuative publications in the framework of the Research Fellowship is to answer above-mentioned questions on the basis of the Balanced Knee by Ortho Development. Therefore 8000 data sets were generated already. 400 data sets of patients with TKA, longer than 10 years implanted, were included as well. To start publicize at the beginning of the Research Fellowship following foundations were laid: .) building of questionnaires for follow-up examination. .) radiographic examination of data-sets (pre- and postoperative alignement) .) updating data sets (Balanced Knee)
