The limited graft survival due to chronic graft dysfunction remains one of the major obstacles of transplantation medicine. Disease processes within the graft are not always reflecting the situation of immune cells in the blood. In fact several projects of the CRC738 have proven that biomarkers from the graft are usually more reliable than biomarkers from the blood. Likewise molecular signatures of tolerance or chronic graft inflammation, graft deterioration and graft senescence can best be studied within the graft. Therefore protocol biopsies are needed to understand the pathophysiology of chronic graft dysfunction, to discover and validate biomarkers allowing an individualized immunosuppression and finally to prolong graft survival.
